February 28-March 1, 2008
Coaches
are requested to print this form, and then ask the
student and their parent or guardian to sign and date it. Additional signatures
are required from you, as the coach, and from the participant’s school counselor
or administrator. Please mail the completed forms to Curtiss Clifton (at the
address below). All forms must be received by January 30th.
